Adopting a simple traffic light system, the Chronic Conditions Assessment Profile is as user-friendly tool that supports managers, HR Business Partners, and OH&S staff, to efficiently and sensitively assess risk and act proportionately.

The Chronic Conditions Assessment Profile weighs risks across 12 key categories at the individual, team, and organisational level, including:

access to medical services and care

workplace accommodations

supervisory rapport, and

business maturity
Once all 12 risks criteria have been assessed, an overall risk rating is automatically calculated and given a numerical value. The risks are ranked LOW, MODERATE, HIGH, and a list of appropriate actions are then recommended to the employer.
